County Awards $750K to Six Food as Medicine Partnerships

Montgomery County's Office of Food Systems Resilience and Office of Grants Management have awarded $750,000 to six partnerships linking primary care providers, nutrition educators, and local farms. The funded partnerships will screen patients for food insecurity and diet-related conditions, then refer eligible households to four-to-six months of medically tailored food assistance. The 12-month program targets at least 450 Montgomery County households with at least one child under 18 who screen positive for food insecurity and a chronic condition such as hypertension, diabetes, or obesity.
